Patient is a 22-year-old female who underwent a previous septoplasty for deviation and difficulty breathing performed by another surgeon. She still had breathing problems, along with an obviously deviated nose that she wanted to make straighter and more attractive. Specifically, she disliked the overprojection of her nasal tip, the obvious dorsal hump and the fact that she felt that her nose started too low, making her forehead appear too long. She desired a nose that better complemented her facial features with more angularity and feminine appearance.
Rhinoplasty and Septoplasty with placement of cartilage grafts to maintain the integrity of the septal repair, reduction of dorsal hump, layered tip graft to improve tip angularity and projection, and a deep temporal fascial graft to the radix (the junction of the nose and forehead) to raise the starting point of the nose, giving improved balance and proportion to the overall result. She was ecstatic with her result.
