This patient is a 20 year-old female who had long desired a more feminine nose that appeared to better fit her facial features. She disliked her prominent nasal hump and wanted her nasal tip to appear softer and more elegant. She was bothered by her small chin and was reminded of her long standing dislike of her side profile when she saw photographs of herself. She had dreamed of a smaller nose that appeared natural and a more proportioned chin and jawline that balanced and accentuated her natural facial features.
Rhinoplasty/Septoplasty with placement of both fascial and cartilage grafts, and chin augmentation. A fascial graft harvested from beneath the patient’s scalp was placed at the top of the nose beneath the eyebrows to soften the takeoff between forehead and nose which serves to make the nose appear shorter and more feminine. The large dorsal hump was removed, and nasal tip cartilages were softened and defined through both reduction and suture techniques. Tip projection was supported with cartilage grafts while the lines of the nose (dorsal aesthetic lines) were recreated giving the nose a better flow and very natural appearance. Lastly, the chin was augmented with placement of a large extended anatomic chin implant that provides volume to the chin and jawline imparting balance to the entire face. She was delighted with her surgical result, and her self-esteem was exponentially increased. Photos were taken only 4 weeks after surgery with still significant nasal tip swelling that will diminish greatly over the next year.
