Patient is a 20 year-old female who underwent a previous rhinoplasty performed in Mexico. She was never happy with her result and saw progressive thickening and bulkiness of her nasal tip that drastically drooped when she smiles. This is the classic “polly-beak deformity” that often happens from poorly planned and executed rhinoplasties that lack adequate support and allow scar tissue to hamper the result. She desires a smaller nose that appears perky, better accentuates her facial features, and that does not plunge with animation.
Procedure: Revision Rhinoplasty with removal of soft tissue scar, reduction and narrowing of nasal bones to create dorsal aesthetic lines, and contouring of the nasal tip cartilages through suture and excisional maneuvers. Her nose was shortened to instill the feminine “perky” appearance she was seeking, and most importantly internal cartilage grafts were placed from nasal septal grafts to provide long-term support and assurance that her surgical result maintains its integrity throughout her life.
