Introduction to Neck Aesthetics
Ideally, a woman has a long, smooth neck that makes her feel and appear graceful. A man’s neck, meanwhile, is support for a masculine jawline that signifies strength.
As men and women age, the skin of the jaw and neck naturally loses strength and tissue begins to sag. In many cases, excess fat and muscle occur, dramatizing this effect. An aging neck can cause the jawline and chin to lose its youthful angle and make the person appear older or even overweight.
A platysmaplasty, or cosmetic neck lift in Scottsdale, restores the jawline, chin and neck to its youthful appearance to produce a proportional chin, a sharp surface joining the neck to the chin and a crisp jawline. Areas addressed include excess skin, excess fat, neck bands or a “turkey wattle” appearance.
A platysmaplasty involves the removal of excess fat and skin from beneath the chin and neck to restore definition and angle between the chin and neck, and the jawline and neck. Dr. Shapiro, a Scottsdale plastic surgeon, will also tighten the neck muscles to restore a sharp jawline. This is typically a very painless procedure.
Patients with good skin elasticity, who are typically younger, may have this procedure performed through a single small incision beneath the chin. For older patients who have a very relaxed neck or poor elasticity, incisions will be located behind the ears to remove excess skin. If a significant amount of skin must be removed, a drain may be placed overnight to prevent a build-up of fluids that may cause complications.
This procedure is typically performed under general anesthesia in an Operating Room setting. If you choose to receive liposuction of the neck, this will be performed first to reduce fat under the chin and neck and remove thickness of the neck or a “double chin.”
Next, muscles are released from the skin and tightened. Skin will be tightened and excess skin removed through hidden incisions under the chin and around the years. This specific procedure is known as a cervicoplasty.
Results will not be immediately noticeable due to swelling, although results will improve as the body heals. Most patients achieve the full, optimal results within six months, although noticeable improvement will be apparent much sooner.
Bruising and swelling after the platysmaplasty surgery are usually minimal, although the neck may feel sore and tight with mild to moderate pain that will be controlled with oral pain medication prescribed by Dr. Shapiro. There may also be numbness around the incision sites for a few weeks. Bruising and swelling should dissipate within a few days. Patients are encouraged to keep the head and neck still following the procedure to allow for healing. The head should also remain elevated during sleep for the first one to two weeks to minimize swelling.
Patients are commonly back to socializing after a week or so.
The Mini Necklift
A mini lift is best for patients who have the beginning signs of aging in the neck, such as loose skin along the jawline, visible banding of the muscles or the start of jowls that decrease definition between the jawline and the neck. Good candidates for this procedure still have moderate skin elasticity of the lower face and neck with a moderately defined jawline. Fat of the neck or beneath the chin is typically addressed with facial liposuction.
Mini Lift Procedure
While the full lift usually places incisions behind the ears or even into the scalp, a mini platysmaplasty requires one incision below the chin. This incision is used to remove excess fat and tighten the muscles with very little, if any, skin removal. This lift will generally tighten the skin as well.
Chin Implant with Platysmaplasty
A chin that is in balance with the face is necessary for a beautiful neck, for both men and women. Many patients who have chosen the platysmaplasty procedure also have a chin implant procedure performed at the same time. This implant can be added through the same incision beneath the chin and complements the results beautifully.

Are You a Candidate?
The ideal candidates for this procedure are in good physical health with fairly good skin elasticity who want to address any of the following issues:
- Double chin or fat under the chin
- Turkey wattle, or loose skin of the neck
- Jowls, or excessive skin and fat under the jaws
- Excessive skin or fat of the neck
- Neck bands
Patients should have realistic expectations and be able to take at least one week away from work and normal activities.
